Foundations ~ 3 month program

Just as a sturdy house requires a strong foundation, so too does your health and wellness. The foundations of health serve as the cornerstone for a life of vitality and happiness. Our Foundations program focuses on these core pillars to help you lay the groundwork for long-lasting wellness.
There is a lot of noise in the health and wellness space, which leaves so many of us feeling confused & overwhelmed. We get it, because we've been there too!  Do you need help getting started?

This program is perfect for you because it will provide you with the essential foundations of health, empowering you to make sustainable lifestyle changes and achieve a state of well-being that can transform every aspect of your life.  Here, you will receive personalized support and accountability surrounding diet & lifestyle. We will work together to develop a personalized plan that fits into your lifestyle and addresses your main concerns.  This program will help set you up on a sustainable, long-term path to thriving.

What's included in the Foundations program?
INITIAL NUTRTION AND WELLNESS CONSULTATION:
During this collaborative meeting we will review your medical history, nutritional assessment, as well as your food & mood journal so that I can give comprehensive, bio-individual recommendations. It is an opportunity to bring to light nagging uncomfortable symptoms, diet & lifestyle habits, longevity goals or desires. If your life has become disrupted by digestive symptoms, cognition issues, mood imbalances, pain & inflammation, poor sleep, or low energy, it’s time to evaluate your current health picture on a deeper level. Based on the findings, we will determine the most optimal approach to help you achieve your goals & relieve your symptoms.  Duration: 90 minutes
RECOMMENDATIONS CONSULTATION:
In this meeting, we’ll discuss imbalances I see that could be related to your health concerns based on my analysis of your paperwork and our initial conversation. I will provide you with my findings, and we will collaborate and work together to design a personalized protocol including nutritional and lifestyle recommendations to help address your health concerns and move you toward your goals.  Duration: 45 minutes
(6) 1:1 PRACTITIONER CONSULTATIONS
A total of 6 sessions to be used within a 3 month period.  This is where we will check in, I will provide you with individualized support, help you set goals, track progress, celebrate your success, discuss any barriers that may have risen and help you make sustainable changes.  It's also an opportunity to answer questions and fine tune your customized protocol so that you can keep moving forward.   Duration: 45-60 minutes.

  • Unlimited contact with me via messaging support - We will help answer questions or provide guidance in between consultations through a HIPPA compliant secure client portal.
  • Access to personalized food, mood and lifestyle journals – to keep you on track outside of our sessions.
  • Follow Up Nutritional Assessment – We will complete a new Nutritional Assessment prior to the end of the program to assess progress over our time together and to determine where you may need further support as you move forward on your health journey. 
  • Long-term Maintenance Plan – During our last session we will discuss nutritional and lifestyle recommendations that can continue to support you on your wellness journey. This will be a sustainable wellness plan for after the program to ensure your long-term success.
  • Access to my online supplement dispensary  - where you will have access to therapeutic, professional-grade supplements (available for purchase separately)**
